Case Management & Outreach Service

Karumah provides confidential, personalised case management and outreach support for people living with HIV across Newcastle, Lake Macquarie and the Hunter region.

Our service is flexible and person-centred. We take time to understand what is happening for you, what support you may already have in place, and what would be useful moving forward. Support may be short-term, practical assistance or more ongoing support depending on your needs and circumstances.

Our case management and outreach service can assist with things such as:

  • understanding and navigating HIV-related services

  • reconnecting with HIV treatment, care or support

  • support with appointments, referrals and service coordination

  • practical assistance during periods of stress, change or difficulty

  • social isolation and connection with community supports

  • housing, financial, legal or other referral pathways

  • support with treatment adherence and medication routines

  • advocacy and communication with health, community or government services

  • support for families, partners or carers where appropriate and with consent

Karumah is not a clinical service and does not replace your treating doctor, nurse or specialist HIV service. Our role is to support you with the social, practical and personal issues that can affect wellbeing, treatment engagement and connection to care.

Support is provided through outreach, which means Karumah does not operate from a public office. Appointments and support are arranged in a way that is private, respectful and suitable to your circumstances.

Referrals

If you would like to know more about Karumah’s case management and outreach service, you are welcome to complete the referral form below.

Self-referrals are encouraged. Referrals can also be made by family members, carers, health professionals, community workers or other services.

If you are referring someone else, please make sure they have given consent for the referral to be submitted. Once the referral is received, Karumah will make contact to discuss the person’s needs, location, eligibility and the support available.
